URS Will No Longer Accept Old Paper Forms » Our new Member Portal offers many online features, such as requesting withdrawals and rollovers. For many other requests, the system generates custom forms. As of June 1, 2026, we no longer accept any old paper forms created before the new Member Portal. Whenever possible, we encourage you to complete transactions online. If you need a form and can't find it online, please contact URS and we will send you the updated version.
